The Submission checklist

Before you hit “send,” ensure your request includes these two mandatory components:

Identity Verification

  • Individuals: A clear copy of your valid government-issued photo ID.
  • Professionals: Submission must be on official agency/firm letterhead or sent from a verified professional email domain (.gov, .law, .org).

Access Authority

  • Standard: A completed Authorization for Release (PDF).
  • Special Cases: If you are not the patient, you must provide the legal documentation (Guardianship, Medical POA, or Heirship) as detailed in your category above.
  • Retrieval Services: A Letter of Authorization from the hiring firm must be included.

  • Processing: Most requests take 14 days.
  • Status Checks: To keep the queue moving, staff will not provide status updates for requests under 14 days old.

Frequently Asked Questions

Most requests are fulfilled within 14 days. To protect staff bandwidth, we will not provide status updates on any request that has been in our system for less than 14 days.

There is no fee for records sent directly to another healthcare provider for “Continuity of Care.” For personal, legal, or insurance use, a standard reproduction fee may apply, subject to applicable laws.

Medical records (clinical charts) and billing statements are handled separately.
